The contribution of the occupational therapist is to maximise the ﬁt between what the client wants to do, needs to do and is capable of doing. Strong et al (1999) have developed an application framework for the Person–Environment–Occupation Model that assists therapists with its integration into practice.
The client is the person who chooses to be involved in the process, as he or she is the one in need of the therapist’s expertise. A therapist engages lawyers, builders or accountants to solve problems related to their areas of expertise and a client comes to a therapist for the same reasons. Everyone expects the people they engage to treat them with respect, to use their expertise to outline the available choices and then to facilitate the necessary actions to implement the chosen options. In client-centred practice the element of choice may be less relevant as the client rarely faces a health or disability issue by choice.
It is unlikely within the healthcare climate of today that a therapist will be permitted to contact the client again in a speciﬁed time to determine whether any further assistance is required. The reality is that, as soon as a client has been discharged from a programme, there is a waiting list of many more to take his or her place.
